Patient privacy

Treatment here is confidential. Information about you or your child can only be released if it is specifically authorized by you with a written release of information form. This release is valid for 90 days and can be revoked at any time at the caregiver’s request. We are required by law to release confidential information in selected cases. This can occur when there is suspected abuse or neglect of children or of dependent adults, danger to self or others, or the inability to meet basic personal needs. In some situations, the court may subpoena treatment records.
